Why it matters

The law also changes oversight of pharmacy benefit managers, or PBMs. PBMs serving employer health plans must pass through 100% of covered rebates and fees, while missing information can bring penalties of $10,000 per day and false information can bring penalties of up to $100,000 per item. Separate Medicare drug-plan compensation and reporting rules begin with 2028 plan years.
Community health centers receive $4.6 billion for fiscal year 2026, with additional funding for the final three months of 2026. Foreign-affairs funding includes $5.4 billion for humanitarian assistance, at least $3.3 billion in military-financing grants for Israel and a temporary prohibition on funding for UNRWA.

Hospitals, laboratories and other Medicare providers
Off-campus hospital departments must use identifiers separate from their main providers and submit status attestations for services starting in 2028. Clinical laboratories face a revised data-reporting schedule and phased payment reductions through 2029.

Agencies must follow detailed spending, notification and reporting conditions. Transportation and housing agencies generally must submit operating plans within 60 days, while many required reports must be posted online within 45 days after Congress receives them.
Foreign governments and international aid organizations
Some assistance is conditioned, withheld or prohibited. The law withholds portions of aid involving Egypt and Pakistan under specified conditions and bars funding for UNRWA through March 25, 2027.
